1. Italy visa application form

You need to provide the following information and documents:

- Papers about personal information:

- 2 new ICAO card photos (not older than 3 months) and unedited, size 4 x 6;

- Household;

- Birth certificate;

- Marriage certificate (if any);

- Documents of the company where you work (depending on the case: notarized copy of business registration certificate, decision to send on a business trip, labor contract, copy of social insurance book, etc.) ;

- Proof of personal financial ability: personal account statement at the bank for the last 6 months, in case of a credit card, a copy of the front of the card and confirmation of the limit is required; Savings account not related to proof of income;

- Travel insurance with a minimum coverage of 30,000 Euro for emergency medical care and repatriation expenses;

- Hotel reservation or proof of payment or free accommodation in Italy;

-  Other documents supporting the visa application may be submitted with the application or additionally requested by the Visa Office.

Note when applying for an Italian visa:

- You must come in person to apply for a visa and take fingerprints.

- Be present at the Embassy or Consulate for an interview (if required).

- For applicants under the age of 18, a parent or guardian must be present at the time of visa application, unless a written authorization from the parent or guardian is required to a third party stating the purpose. Authorised.
